My son/daughter, ______, is a _____ grade student attending ________ School. _____has been recently diagnosed with childhood onset bipolar disorder. Bipolar Disorder (manic-depressive illness) is a treatable neurobiological dysfunction of the brain that is characterized by extreme changes in mood, energy, thoughts and behavior. Children with bipolar disorder often demonstrate executive dysfunction with difficulty staying on task, decreased impulse control, organizational difficulties, decreased and fluctuating cognitive ability, and decreased functional ability - all of which serve to decrease the child's availability to the educational process. In addition, children with bipolar disorder are at increased risk for school failure, substance abuse, and suicide.
I formally request a multidisciplinary evaluation to determine the extent of ______ educational disability and determine his/her eligibility for special education and related services, as defined in federal IDEA statutes. Thank you for your timely attention to this matter.

REQUEST FOR A FUNCTIONAL BEHAVIORAL ASSESSMENT

IDEA requires a Functional Behavioral Assessment be completed in preparation for the IEP team to meet and write a Behavioral Intervention Plan (BIP) to address my child's' specific needs. We/I formally request that a Functional Behavior Assessment be performed.
In addition, we request a copy of the FBA assessments prior to the meeting so we may be able to review and assist the IEP team in creating our child's BIP.
